Supported Living

Supported living is a community-based service where you may have carer support 24 hours a day. the primary aim for this type of support is to enable you to become as independent as possible, supporting you to learn and improve upon existing life skills. You may rent your own house or share a property with communal spaces with other people. We'll make sure resident match and benefit from each other.

We will help you to understand the tenancy agreement and the rules as well as helping you to apply for support applying for housing benefits.




Our Approach to Supported Living


Our Supported Living services include:

  • Person-centred care planning: comprehensive assessment, individualised care plans and regular reviews and adaptations
  • Building trust and rapoort: establishing trust and active listening
  • Effective communication: clear and accessible communication adapted to our service users and consistency.
  • Encouraging self-advocacy: empowering our service users.
  • Promoting skills development: regular skills assessment and incremental challenges plan.
  • Community integration: engagement in local activities and promoting social connections.
  • Emotional support: our professional team will work mental health awareness, emotional resilience and self-acceptance.
  • Collaborative care: we work with multidisciplinary teams and family involvement when possible.
  • Crisis intervention and prevention: plans are in place to respond to emergencies and challenging situations. Staff are trained in de-escalation techniques.
  • Documentation and record-keeping: accurate and detailed records are maintained as well as privacy and confidentiality.

We will also work collaboratively with local authority partners, NHS services, therapists and other community bodies to ensure that our support fits in perfectly with the overall healthcare you are receiving.
