The Dementia care plan might include:

• Reminding or assisting you with taking medication
• Helping with with bathing/dressing to keep you looking your best
• Catheter care and continence management to maintain your dignity and comfort
• Helping you keep active and mobile to improve mood and mobility and to strengthen muscles
• Helping you with nutritious and tasty food choices to keep you healthy and avoid constipation
• Assisting with routine household chores and shopping so your home life is as you would like it to be
• Companionship to keep you safe and engaged at home, or helping you to be involved in social activities.

We will also work collaboratively with local authority partners, NHS services, therapists and other community bodies to ensure that our support fits in perfectly with the overall healthcare you are receiving.
